Welcome to the Bean Creek Survey

The survey consists of 17 questions. Each question can have multiple required responses. At the end of the survey is a "Submit Survey" button. When you have completely  filled in your responses, simply click on the button. If you omit any answers, a message will direct you to use your browser's BACK button to make the correction.
